PEARL
Prospective Evaluation and Assessment Registry for Liver disease
A national, multicenter, prospective registry tracking the epidemiology, treatment patterns, and clinical outcomes of Metabolic dysfunction-Associated Steatotic Liver Disease (MASLD) across Saudi Arabia, benchmarked against the European LITMUS Registry and TARGET-NASH program, governed by the SGA.
Registry Objectives
- Establish a comprehensive national database for MASLD/MASH patients across Saudi Arabia, addressing the 30%+ prevalence in the population
- Characterize MASLD phenotypes, metabolic comorbidities, and genetic risk factors in the Saudi population
- Track non-invasive test (NIT) performance including FIB-4, ELF, VCTE, and novel biomarkers against biopsy-confirmed staging
- Monitor treatment outcomes for lifestyle interventions, GLP-1 agonists, resmetirom, and emerging pharmacotherapies
- Build a biobank linked to clinical data for translational research and precision hepatology
- Enable collaborative multicentre studies benchmarked against LITMUS, TARGET-NASH, and NIMBLE standards
About PEARL
PEARL aims to establish the first comprehensive, prospective national database for Metabolic dysfunction-Associated Steatotic Liver Disease (MASLD) in Saudi Arabia. With a prevalence exceeding 30% in the Saudi population, MASLD represents the most common cause of chronic liver disease in the region.
Benchmarked against the European LITMUS Registry (the largest international biological data registry with 10,000+ patients), the US TARGET-NASH program (largest real-world cohort), and the NIMBLE biomarker validation initiative, PEARL represents the first comprehensive national MASLD registry in the region.
Regulatory Compliance
PEARL complies with the Saudi Personal Data Protection Law (PDPL) and is HIPAA-aligned. All data is encrypted and monitored with full audit logging. The registry includes a biobank component compliant with SFDA standards for biological sample storage.
